How it Works
The Self-Pay Program is designed to minimize cost in order to make quality healthcare obtainable without health insurance. At Epic, an affordable and comprehensive treatment plan is personalized for each individual.
Office Visit
- Simple Urgent/Family Care Visit – $140
- School/sports/camp PE (< 19 yr) – $70
- Pre employment physical ( >19 yr) – $120
- DOT exam (Driving Medical Card) – $110
- Immigration Physical – $350
- Well Adult Check up with Lab – $195
- Well Woman Exam with Pap Smear (includes HPV testing) – $195
- Comprehensive STD Testing – $300
Common In house labs / Procedures *(add-on to the office visit)*:
- Rapid in house swab test ( Flu, Strep, Covid, RSV )
- Urine test/Urine culture
- TB test QuantiFERON -150$
- TB skin test- 60$
- EKG – $75
- X Ray – $90
- Additional laboratory tests – discounted prices available. Please check with clinic staff.
Common Vaccines***:
- Tdap – $70
- Varicella – $240
- Hepatitis B ( Adult) – $120
- MMR Vaccine – $140
- Flu vaccine – $45
*** Uninsured pediatric patients between age 0-18 years old will be eligible for VFC (vaccine for children program). Cost of vaccine administration will be 20$ for each vaccine. Office visit charges will be applied
Promotional Services:
Comprehensive Wellness Exam – $195
Includes exam, Urine Test and Blood Tests (lipid profile, complete blood count, kidney profile, liver profile, diabetes test (Hemoglobin A1C), thyroid test)
Well Woman Exam( Pap Smear) – $195.
Includes pelvic exam, breast exam, PAP smear with HPV
STD Check up ( Blood + Urine) – $300. I
ncludes HIV 1-2/ Syphilis/Hep B testing /Hep C testing/
Urine chlamydia and Gonorrhea, Trichomonas testing
Immigration Physical Exam – $350.
Includes physical, Labs, sealed/signed envelope by USCIS approved Civil Surgeon (exclude vaccines and chest X ray)
*Self-pay services are offered for anyone paying without using insurance. Participation in self-pay pricing cannot be used in combination with commercial, federal or State insurance benefits.
*Additional charges may apply due to complications, multiple concerns, complex conditions or unforeseen circumstances, such as: complex procedures (extensive lacerations), outside lab tests, medications, injectables, etc. Level of care is determined by Epic Urgent and Primary Care
